A template gets you started quickly. Below are examples from the four templates in the survey builder at DarmoweSondy.pl. This is a preview of the content only: answers are not collected on this page. After you create a survey from a template, you can change the questions, the answer options and the design.
Yes / No poll
Question: “Do you support this idea?” Answers: Yes, No, No opinion. Replace “this idea” with a specific proposal so that every voter is judging the same thing.
It works well for a quick vote. See how to embed a poll on your website.
Customer satisfaction survey
- “How likely are you to recommend us to a friend or colleague?” – a scale from 0 to 10 (the question behind the Net Promoter Score, NPS).
- “Please rate the following parts of our service” – a table covering delivery time, communication, product quality and value for money.
- “How often do you use our services?” – a single-choice question.
- “What could we improve?” – an open question.
Say in the description which service or visit the answers refer to, and delete any item the respondent could not have rated.
Training or event feedback
- “How would you rate the training overall?” – a scale from 1 to 5.
- “The topics were useful to me” – a statement rated on an agreement scale.
- “The trainer explained things clearly” – a separate rating of the delivery.
- “Your comments and suggestions” – an open field.
Send the link or show the QR code after the event. Explain that the answers will help you prepare the next session. For feedback during the session itself, try a live poll.
Thesis survey
The multi-page template includes an introduction, a demographics section, a table with a Likert scale, questions about frequency, multiple choice and open fields. The placeholder statements must be replaced before you publish. The template is a starting point and does not replace checking the methodology of your study.
See a sample introduction and a guide to preparing a survey for your thesis or dissertation.
Tips for adapting any template
- Delete every question you will not use. Respondents are more likely to finish a short survey.
- Replace the placeholder wording with your own: the name of your product, course or event.
- Keep one idea per question and label both ends of every scale.
- Test the finished survey on your phone before you share it.
How do you use a template?
- Create an account and confirm your email address.
- Start a new survey and choose the template that fits.
- Adjust the title and the questions to your purpose.
- Check the preview and the settings, then publish the survey.
